IDENTIFICATION
The vehicle involved is a 1994 Wheeled Coach Type III modular ambulance body remounted on a 2003 Ford E-350 SD chassis-cab, equipped with a Ford 7.3L 185HP diesel, 3-speed automatic trans, 4,600# twin-I beam front end, dual wheel 7,500# single drive axle with spring suspension + Ride Rite air bag helpers, 138” standard wheelbase. VIN: B57621 was (was not) verified as assigned. A vehicle identification form is enclosed, plus a note sheet for ambulance equipment.
DAMAGES
Damages were sustained to the front & RH body sides, doors, inner structure, entry door opening, cabinets, etc. because of collision impact. Detailed digital photos are attached.
